Job Description

  • Under the direction of the Clinical Director, the Community Support Supervisor is responsible for management, supervising, and administering the programs, staff and fiscal operations of the Community Support Program.
  • Provides clinical and supervisory direction for all staff assigned to this position
  • Supervises and evaluates the work of assigned Community Support Specialist
  • Participates in the recruitment, interviewing, and selection of Community Support Specialist staff
  • Provides orientation and training to new and existing Community Support staff on program procedures and regulations
  • Oversees a quality assurance program for Community Support Specialist care management activities
  • Conducts case reviews to ensure compliance with program rules and regulations
  • Identifies trends and determines appropriate actions to correct care management errors

Independent Living Skills Trainer is a teaching service. Specific activities may include assessment and training in: self-care; medication management; task completion; communication and interpersonal skills; socialization; sensory/motor skills; mobility and community transportation skills; problem solving skills; money management; and the ability to maintain a household. Assistance and supervision are not provided under this component. Employed staff must:

  • Be at least 18 years of age
  • Have a minimum of a Bachelor’s Degree and one year’s experience providing services to individuals with brain injuries in the community

AND

completed training program(s) concerning acquired brain injury (ABI) and person-centered planning, given by a State Agency, Allied Community Resources, Community Providers, Brain Injury Association of Connecticut, or Independent Living Center

OR

  • Demonstrate ability to function as a member of an interdisciplinary team
  • Have documented experience implementing cognitive/behavioral interventions developed by a clinician and utilized in community settings.

Job Description

  • Provide supervision and socialization that are provided in accordance with a therapeutic goal included in the service plan and not purely divisional in nature. The companion may assist in or supervise such tasks as meal preparation or laundry, but does not perform these activities as discrete services. The companion may also perform light housekeeping tasks that are incidental to the care and supervision of the individual. Job Summary:

Peer Support is an ongoing service and supports designed to assist participants to acquire, maintain or improve the skills necessary to live in a community setting.  This service provides supports necessary for the participant to develop the skills that enable them to become more independent, integrated into, and productive in their communities.  Peer Support includes face to face interactions that are designed to promote ongoing engagement of persons covered under the waiver in addressing residual problems resulting from psychiatric and substance use disorders, and promoting the individuals strengths and abilities to continue improving development of natural supports, and maintenance of community living skills.  Duties include but are not limited to: Assistance in daily management, social and emotional support, linkages to clinical and community resources, ongoing support keeping clients engaged by providing, flexible and continual long-term follow-up. This service enables the participant to retain or improve skills related to personal finance, health, shopping, and use of community resources, community safety, and other adaptive skills needed to live in the community.

ESSENTIAL DUTIES AND RESPONSIBILITIES: include the following. Other duties may be assigned.

  1. Be at least 18 years of age

  3. Have a minimum of a one year’s experience providing services to individuals with brain injuries/Mental Health/Addiction Recovery in the community.
